Full Job Description

Use their initiative and take responsibility for themselves and the quality of their work and the service they provide to patients. Work as part of the Multidisciplinary team across a range of clinical pathways. Manage a caseload of patients with complex needs, using evidence based / patient centred principles to assess, plan, implement and evaluate interventions; these interventions will be underpinned by NICE and Best Practice Guideline. To purchase packages of care where required. Support patients and carers to achieve agreed outcomes and goals utilising the recovery model. Provide leadership for junior staff, through supervision and appraisal. Participate in the planning, development and evaluation of Services, within a designated area / team, holding responsibility for
