Full Job Description

We currently have a permanent vacancy for a full-time band 5 nurse to work in the Older People's Community Mental Health Team in Heywood, Middleton and Rochdale (formerly known as the MERIT Team).

The team is comprised of community mental health nurses, an occupational therapist, team manager and a team administrator.

The service aims to meet the needs of people experiencing serious mental health problems in the community who are over 65 years of age and people under 65 who are diagnosed with early onset dementia, as well as their families and carers.

We are looking for an enthusiastic, driven nurse, who is committed to working with this client group and being an integral part of our multidisciplinary team., To undertake the role of care co-ordinator in the assessment, treatment, and delivery of care to older people with serious mental health problems, who meet the criteria for access to the service. You will ensure that the highest quality care is carried out in accordance with the policies and procedures of Pennine Care and the Nursing and Midwifery Council.
