Full Job Description

We are looking for a highly motivated individual who is passionate about working with individuals living with dementia, promoting the concept of living well with dementia. You will need to hold excellent communication skills, be enthusiastic and flexible in your approach, and demonstrate an understanding of the complexities involved in this specialist area. We are a countywide service that provides assessment, diagnosis and a comprehensive service across the pathway for people diagnosed with dementia, providing signposting, advice, education and support to the person living with dementia and their families/carers and loved ones. As part of our team, you will be working autonomously and have a varied role allowing plenty of scope to both demonstrate and develop your skills. You will work closely with and be supported by our multi-disciplinary team having access to the support of admin, healthcare health support workers, nurses, psychologists, consultant psychiatrists and occupational therapists. Whilst working alongside the immediate team you will also work with our wider health and social care community, in particular our colleagues in general practice, care homes, social care, admiral nurses and our 3rd sector services such as the Alzheimer's Society. Although based within a specific area there will also be an expectation to travel within the geographical area to support the service as necessary.,

  • To provide specialist support to individuals living with dementia
  • Assess mental health, physical health and social problems to ensure appropriate treatment, support and management
  • Undertake a comprehensive assessment in conjunction with a more senior member of the team
  • To deliver a range of interventions in accordance with the agreed plan of care
  • Complete and contribute where appropriate to robust risk assessments and risk management plans
  • To work as part of a multi-disciplinary team
