Full Job Description
Are you an innovative, creative, motivated caring professional who is looking for a new challenge? We have an exciting opportunity to join our well established Weymouth and Portland Community Mental Health Team.
As a Mental Health Practitioner you will act as a Care Coordinator for a dedicated caseload of patients suffering with Serious Mental Illness with whom you would provide person centred, evidence-based, targeted interventions to aid recovery. You will have a working knowledge of current bio psycho-social models of care and understand how to implement such interventions with your patients to promote optimum health and wellbeing. You will be provided with regular clinical and peer supervision.
The team work collaboratively, you will have the support and opportunity to develop your own skills in evidence-based therapeutic interventions in the psychological therapies as well as advanced nursing., The Mental Health Practitioner role is a Full Time permanent position, and covers the Weymouth and Portland area. You will need to have a valid driving licence and use of a car as you will be required to review and monitor patients in their own homes and within other community settings. Opportunities for flexible working are available as long as the service needs are met., You will work closely within our Integrated Multi Disciplinary Team that consists of Consultant Psychiatrists, Mental Health Nurses, an Occupational Therapist, Social Workers, an Advanced Nurse Practitioner, Support Time Recovery Workers, a Physical Health Worker and our Psychology Team. There will be a good deal of work with multiple professionals across a wide range of services.
You will have the opportunity to act as a mentor, assist with induction and in-service training of staff when necessary. As a Mental Health Practitioner maintaining and developing effective links and sound communication with the Inpatient and Crisis Service, particularly in respect of aftercare of discharged patients and ensuring good communications with relatives and other visitors.
