Full Job Description

The REACT team offers interventions which; are focused on patient goals and outcomes, are based on what support they want and ensure there is 'no decision about me without me.'

The team emphasis is mental health assessment, risk assessment and management, person centred care, psychotherapeutic interventions and collaboration and joint working with other mental health services within MHSOP.

The REACT Team has a full-time Band 6 position available for an experienced and dynamic Mental Health Nurse to join their Team., Key nursing skills and knowledge; patient-led focus / mental health assessment, care and treatment/ psychological interventions/ autonomous working.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

REACT is a multi-professional community team based in Llanfair Unit, Llandough Hospital and it sits within Cardiff and Vale University Health Board's Mental Health Services for Older Peoples' Directorate.

The team operates seven days a week; Monday to Friday 08.00 to 20.00hrs and on weekends and bank holidays 09.00-17.00hrs.

The REACT team offers a rapid assessment response and intensive home treatment for people who are experiencing acute and/or severe mental health distress or where care at home is at risk of breakdown.

REACT's aim is to work with people;

of any age with dementia

with people over the age of 65 years who have a late-onset mental health condition

as well as their families

providing an alternative to hospital admission for people who are best supported at home, promoting appropriate access to MHSOP inpatient beds when this is required and supporting people with timely discharge from hospital back into their community settings., Cardiff and Vale University Health Board is one of the largest Integrated Health Boards in the UK, employing over 17,000 staff, providing over 100 specialist services.
