Are you a qualified mental health nurse looking for a new challenge?

The Hertfordshire Adult Mental Health Community Transformation Programme is implementing new mini teams within Adult Mental Health services to focus support and give more time to care for service users with low intensity psychosis - a new Low Intensity Psychosis Pathway.
This Band 6 post will be the named nurse lead for the new mini team in a specified Adult Mental Health Service, working closely with the Clinical Nurse Specialist and the MDT of the service to support this new focus.
The Low Intensity Psychosis Pathway seeks to promote the personalised recovery and future care of those service users who are settled in mental health, and have an enduring diagnosis of psychosis, by working in partnership with people with psychosis and their carers to:
+ offer help, treatment, and care in an atmosphere of hope and optimism
+ take time to build supportive and empathic relationships as an essential part of this care., To lead a mini team of staff to oversee the holistic care of service users on the Low Intensity Psychosis Pathway.
To carry a clinical caseload and fulfil the named nurse role.
To undertake comprehensive health needs and risk assessments
(including capacity assessments) and develop a care plan in accordance with identified needs establishing specialist
course of responsibilities/actions based on Recovery Principles.
To carry out joint assessment with other MDT members as required.
To be aware of and comply with the relevant mental health and work
related legislation and policy at all times.
To provide relevant up to date evidence based information about
treatment/intervention that enables service users to make informed
choices about their care.
To deliver evidence based care in a timely manner.
To make reactive recommendations as necessary when working alone
in the community.
