Community Mental Health Nurse ADAPT Team

We have an exciting opportunity for a nurse to join the Bromley East ADAPT Team. The successful candidate will have the opportunity to work in a busy and varied multi-disciplinary environment. You will work alongside clinical and other non-clinical colleagues to make a positive contribution to our patient's journey to recovery., The post holder will be responsible for undertaking comprehensive assessment of service users and carers mental health and social care needs. Once these needs have been identified the post holder will be responsible for developing a goal focussed and person centred care plan to enable the provision of an appropriate intervention.
