Full Job Description

The post holder carries responsibility for the assessment of care needs and the development, implementation and evaluation of programmes of care for service users who are referred to the recovery and wellbeing team. They will be a member of a multi-disciplinary team and work in close co-operation with hospital based and primary health care based professionals. They will be responsible for the maintenance of standards of care within the Recovery & Wellbeing Team and actively participate in the training and education of students and staff seconded to the service. In addition, they will also be responsible for ensuring that an integrated assessment and treatment service is provided to the clients and ensure that a good inter-disciplinary working environment is maintained.

1. To establish and maintain effective individualised care using the Care Programme

2. To participate in team meetings, initiating and discussing new methods/models of care and participate in the introduction of research or trails within the area.

3. Seek to establish a therapeutic relationship with individual clients and their families.

4. Ensure effective and confidential communication between all care professionals.

Provide significant data governing client care and treatment by:-

a. Participating in informal or formal case conferences and discussions.

5. To develop close links with relatives/carers and ensure that effective communication takes place especially when clients are admitted or being prepared for discharge from hospital and that all appropriate agencies are informed, so that follow up care can take place.

6. To advise the manager and Professional Nursing Advisor on all breaches of nursing professional conduct as required by the Nursing and Midwifery Council's Code of

Professional Conduct.

7. To liaise with the Manager to ensure that all (Eput) Essex Partnership University

Trust policies and procedures are communicated, implemented and developed within the area.

8. To participate in professional supervision on all allocated cases.

9. To take on clinical work commensurate with experience and skills, in discussion with supervisor, and senior staff.

10. Any other duties as appropriate to the grade as may be requested e.g. Duty., b) Planning, implementing and reviewing interventions designed to meet assessed needs.

c) Providing advice and information to service users, their carers and their families in order to alleviate distress and work in partnership, as far as possible, with all the available support systems.

d) Providing clear, accurate and concise documentation of the service user's care as required by the Trusts Care Programme Approach (CPA) Policy.

c. Establishing regular written reviews of progress

d. Ensure that information is recorded on the client information database.

5. To develop close links with relatives/carers and ensure that effective communication takes place especially when clients are admitted or being prepared for discharge from hospital and that all appropriate agencies are informed, so that follow up care can take place.

6. To advise the manager and Professional Nursing Advisor on all breaches of nursing professional conduct as required by the Nursing and Midwifery Council's Code of Professional Conduct.

10. To liaise and work in partnership with the voluntary sector, maintaining a close relationship for the benefit of our mutual client group.

11. Out of hours and weekend working may be required according to the needs of the service,

12. Any other duties as appropriate to the grade as may be requested e.g. Duty.

ADDITIONAL DUTIES

In addition to the above duties you will also be expected to perform the below key activities in line with your job role;

- Complete mandatory training in line with Trust policy and procedures

- To participate in the staff appraisal process and to undertake for any staff you manage

- To keep yourself updated on all matters relating to Trust policy

- To provide management supervision where appropriate

- You will be expected to work collaboratively with key partner organisations, service users, carers, clinicians and other practitioners within the multi-disciplinary team in delivering services; providing a 7-day a week, 24-hours a day service for 365 days a year working shifts, where appropriate and operationally required.
