If you are passionate about working with older adults and their families, then we want to hear from you! We are looking for a highly motivated, enthusiastic Mental Health Practitioner.

An exciting opportunity has arisen within The New Forest West Older Persons Mental Health Team for a Band 5 Mental Health Practitioner. As a community MHP your journey will take you around the beautiful New Forest and along the coast delivering high quality care and interventions to our patient group. We are a well-established team offering support to older adults normally aged 65 or over unless presenting with early memory deficits who are experiencing mental health problems. This might be due to an organic mental illness or a functional mental illness. There are opportunities to be involved with our memory assessment service and inreach working to our local care homes. We also have close links with research studies and other community services.

The supportive leadership team promote the development of our services to provide the best outcomes for our patients .We know to achieve this we need to invest in our staff through creating a good working environment with training opportunities. We are also open to flexible ways of working with hybrid working a mix of face to face and online/telephone assessments to help balance your work and home life., Carrying a Clinical Caseload and contributing to the process of patients ongoing assessment and formulation of person-centred care plans.

To work in partnership with families and carers to ensure the patient's individual health needs are met.

Working with the multidisciplinary team to ensure safe patient care and individual needs are met and clearly communicated amongst those involved in their care.

Delivering community interventions and signposting to other services as appropriate.
