Community Mental Health Practitioner (AA&HTT) | Essex Partnership University NHS Foundation Trust

As part of this developing service you will help support older adults with functional mental health issues to meet increased needs / rising risk to support admission avoidance and provide home treatment with a Home First approach to support. You will work with a new and growing team, the existing community services and the established Dementia Admission avoidance services across Mid and South Essex

Band 6 -£35,392 - £42,618 per annum (pro rata for part time hours) Functional Intensive Response and Support Team The Crystal Centre Part Time Hours 22.5 hrs per week. As part of the transformation agenda a new service is being formed to provide support to Older Adults, with Frailty and Functional Mental Health needs.
