Full Job Description

The successful applicant will work effectively with adult service users with mental health needs across Bury. The post holder will be highly skilled and competent in both holistic health assessment and identifying social needs, risk management and Formulation, developing collaborative care plans, facilitating the implementation of the agreed plan for care with a focus on recovery and social inclusion. The post holder will provide a range of interventions and the opportunity to develop these within the role., The Practitioner will provide comprehensive mental health service to adults of working age who experience moderate to severe mental health problems in a community setting.

-Utilise the Care Programme Approach to support and promote the recovery of service-users under the care of the service.

-Working collaboratively across a range of disciplines to continuously promote a needs based, recovery focused approach to all service-users.

-Work within, and promote, an integrated approach to service delivery.

-Develop clear relapse recognition/prevention plans in a collaborative manner and share with MDT, service-users and carers where appropriate.

-Develop meaningful engagement with service-user's and carers.

-Offer support as needed as part of Duty Rota.

-The Team offer a Monday-Friday service within the hours of 9am-5pm.

-Flexible working can be discussed in line with Trust Policy.

An exciting opportunity has arisen for a passionate and enthusiastic Mental Health Practitioner to work within the Bury Community Mental Health Team. The successful candidate will be joining a dynamic Multi-Disciplinary Team at an exciting time of positive change.

The post holder will be expected to demonstrate high level of commitment, motivation and knowledge of working with people with severe and enduring mental health problems. The role will require clinically effective assessment, care management skills and interventions in accordance with the Care Programme Approach and Care Act framework.

The post holders duties will include engagement with clients in the development of person centred, recovery focused care plans, supporting our experienced duty team with short-term pieces of work and supporting people managing crisis. You will need to demonstrate excellent communication skills and be able to work collaboratively in a multidisciplinary setting. You will be dedicated to delivery, both as part of a team and working independently, of a high-quality, innovative, recovery focused and person centred care which meets the needs of people who use our service and their carers.
