Full Job Description
You will be responsible for the assessment of care needs and contributing to; the formulation of the person's needs and the development of a relevant plan based upon the person's negotiated goals and relevant evidence based interventions.
You will, in conjunction with the multi-disciplinary team, provide therapeutic interventions to support the person to meet their agreed goals and coordinate and/ or participate in the review of progress towards those goals, utilising recommended outcome measures, involving the service user and any identified carer throughout the clinical pathway., The Integrated Mental Health teams work as part of a 'neighbourhood' offering place based provision of mental health services in partnership with other local organisations. Clinicians within the IMHT will offer interventions and treatments which are concordant with NICE guidelines, and allocation within the team will be based on the needs of the service user, matching clinician skills to service user needs.

The usual hours of work are Monday to Friday 9am - 5pm. You will also be required to travel over large geographical area so you must have access to suitable transport.
