Full Job Description

Working within the treatment function of the AMHT you will co-ordinate the care for patients with severe and enduring mental health diagnosis. You will need to be able to work autonomously, with a caseload of patients with whom you will engage and offer the recommended treatment and support. You will have experience of providing community mental health care and able to assess and plan treatment with service users. You will also be able to assess and manage risk, including use of Safeguarding processes. There are opportunities for team working and facilitating groups such as MBT/CBT based interventions. The MDT consists of medics, psychological therapists, social workers, OTs, RMNs and support workers, as well as embedded workers from the third sector to support with employment, housing and social needs. In addition we have embedded Peer Support Workers within the teams to promote the power of lived experience and ensure that the gap between patient and clinician is bridged. The post holder will need to be able to travel around the county, to offer interventions to patients and carers. The area is quite rural with limited public transport, so access to transport is essential., We are a supportive, dynamic and integrated team which although busy has many rewards. The team is highly supportive and committed to delivering high quality patient care; where the caseload can be demanding, diverse and frequently complex. Regular supervision will be provided and you will be encouraged to undertake any training relevant to your professional and personal development. You will be supported by an experienced leadership team from across the professions who facilitate forums for Nursing and Occupational Therapists and hold your development as central. Our AMHT provide an assessment and treatment function and the staff working within the team work on a rota covering shifts from 07.00 to 21.00 (7 days/week). The successful candidate will be required to work mainly 9-5 Monday to Friday however there is an expectation that there is flexibility to cover the out of hours shifts when required. The team are working using the FACT model of care. There are daily triage meetings with all members of the MDT (Doctors, social workers, nurses, occupational therapists and psychologists). + All new starters have a thorough induction process, both local and Trust wide, which aims to create a positive supportive working environment allowing new employees to learn key elements of their role over a reasonable timescale. + Appointment to this post is subject to the trust receiving satisfactory references covering 3 years of employment or study. This role carries an incentive premium of £3,500 which will be paid in instalments over an 18-month period

We would consider full time (37.5 hours a week) or part time (minimum of 30 hours a week)
