Full Job Description

We're looking for a talented health or social care professional to join our leadership team in Dacorum North West Hertfordshire as Service Manager. The role will provides a genuine opportunity for you to develop your management skills and make a difference. The post is an 8 month secondment. You'll have the opportunity to participate in our Leadership Academy, and benefit from excellent promotion prospects, some state of the art clinical environments. We welcome diversity in our workforce and encourage applicants from people of all ages and backgrounds, such as those with lived experience of mental health difficulties., You will work closely with the Service Line Lead and management teams to ensure the effective co-ordination and operational delivery of community health & Social care services in HPFT. Particular key responsibilities will be to:

  • You will assist the Service Line Lead deliver a service taking account of the Trusts transformation agenda and implementation of care pathways ensuring a cohesive, effective and seamless service is provided for service users.
  • Ensure the effective delivery of community based care services working on a whole systems approach to achieve required levels of performance.
  • Support management of relationships with primary care general practitioners, HCS, PCT and 3rd Sector organisations as required by the Service Line Lead, Responsible for the provision of the quality care within a complex range of health and social care services that interface across HPFT and with other healthcare providers.
  • Accountable for the effective operational management of all care pathways, requiring effective liaison and co-ordination with secondary care clinicians, managers, local politicians and primary care and other independent contractors for Community Services.
  • Manage within the established frameworks and processes by which the Strategic Business Unit will manage and monitor capacity on a day to day basis, ensuring effective escalation arrangements are in place to maintain safety with the ability to respond where team capacity issues present and manage the development and delivery of team health promotion and prevention, working closely with other agencies.
  • Ensure compliance with all statutory requirements e.g. CQC, HSE and implement relevant assurance systems to demonstrate compliance for their service

    First or higher degree
  • Professional qualification in relevant area of health or social care
  • Postgraduate qualification or formal management training., Demonstrable success at managing change and development
  • Demonstrable success in quality improvement
  • Significant management experience in relevant area of health or social care
  • Demonstrable success in performance management and appraisal with multi- disciplinary staff group, Ability to organise and prioritise a complex workload
  • Ability to produce clear, written statistical and financial reports.
  • Ability to communicate effectively at all levels of involvement including Service Users and Carers
  • Ability to influence and motivate staff to engage with and adopt change and deliver challenging targets, Ability to analyse critically financial performance and clinical data sets and present conclusions
  • Innovative thinker with ability to recognise underlying causes of and overcome barriers to change, You must have appropriate UK professional registration.
