Full Job Description

  • Engage directly with our customers to develop personal, tailor-made support plans that reflect their aspirations and needs.

  • Lead and coordinate multidisciplinary teams during MARAC, safeguarding, and other multi-agency health meetings to ensure cohesive support strategies.

  • Advocate passionately for your customers, ensuring their access to necessary community resources and services that align with their goals.

  • Monitor and adapt support strategies, ensuring they remain aligned with individual goals and are responsive to changing needs.

  • Use your strong influencing and negotiation skills to guide customers towards making beneficial decisions for their mental health and wellbeing.

  • Maintain meticulous records of customers? progress, applying agreed screening processes for eligibility and referral with precision and care.

  • A deep understanding of and empathy for individuals? life stages and challenges, paired with an exceptional ability to listen and adapt to diverse needs.

  • Proven experience in creating and implementing effective, person-centred support plans.

  • Robust coordination and assessment skills, capable of managing referrals and ensuring seamless service delivery.

  • Strong interpersonal skills to work effectively within a team and lead by example in mentoring and sharing knowledge.

  • An eye for detail with a commitment to working within legislative frameworks and organisational policies.

  • A resilient nature, capable of working independently in dynamic environments, and an unwavering passion for supporting others to live their best lives.

    7 Month Fixed Term (with possibility of being extended), full or part time job share (37.5 hpw), Mon - Fri 9am-5pm

    Pay £24,664 per annum (pro rata) and great benefits including Health Cash Plan

In this vital role within the Durham Mental Wellbeing Alliance, you'll connect service users with essential resources, empowering them and promoting positive mental health outcomes. It?s a chance to truly enrich lives, including your own, through meaningful interactions and dedicated support.,
  • Initially you would be in post for 7 months but there is the possibility that this will be extended.

  • Working Monday to Friday 9 am to 5 pm.

  • Your main base being in Seaham Safe Harbour.

  • Our service works with customers for 4-6 sessions making sure they are connected to the relevant services they need.

  • The alliance covers the whole of County Durham, the community navigator will have a dedicated patch/area to cover within the County.
