Do you have the skills to work in community physical health? Are you interested in working for a dynamic and innovated trust who provide person centred health care to our local community patients? We'd love you to join our teams.
We have exciting opportunities for Band 5 enthusiastic, motivated, and keen nurses to join our established multidisciplinary Care Community Teams to support and deliver a range of quality nursing services to our patients.
Your skills will be really valued by our teams and patients alike. If you are new to community health, this is a great opportunity to gain experience in this unique field of nursing.
We will do our best to welcome you and help you to settle in as a member of the team. Your contribution, time, skills and caring will make a huge difference to the patients we care for.
There are a wide range of posts available on a full time, part time or temporary basis, across the care community teams., Our eight care community teams are based in various locations within Chester, Ellesmere Port, Neston, Frodsham and the Rural locality covering the geographic area of West Cheshire. It's important that you can drive and have access to your own vehicle for work. But, of course, mileage expenses are reimbursed in addition to your hourly rate.
As a Community Staff Nurse the successful candidates will undertake duties involving the delivery of high-quality nursing services and the holistic assessments of patients ensuring a person-centred approach.
You will work alongside various professionals to provide quality and seamless care to patients predominantly in their own home. You will be proactive in coordinating their care needs as appropriate, avoiding unnecessary admission to hospital or long-term care settings.
