Full Job Description

This is an excellent opportunity for a highly motivated and enthusiastic Band 5 Community Staff nurse to join the Radcliffe District Nursing Team , working flexibly over a seven day period to cover 08:30 to 17:00 Hours or long days over 08:00 - 20:30., We are part of a dynamic, integrated health and Social multidisciplinary team delivering high quality care, advice, and support and monitoring to patients, their families and staff within the community settings. You will be involved with the development of INT working and leading on new initiatives to prevent admissions and facilitate discharge. You will work under the direction and supervision of the team leader to provide care to patients with complex and non-complex needs with the aim of promoting independence and self-management.
