We are seeking either newly qualified and or experienced nurses to provide complex care and treatment to adults living in the area who have long-term or complex health needs..

To support you within your role you will have access to further develop your clinical skills and leadership development are available and encouraged. We are committed to support the development of your career and have a competency-based approach to clinical training and development, which will ensure you are able to apply your newly developed skills in a safe and effective way.

Main duties of the job

Participate in the holistic assessment, implementation and evaluation of programmes of care for patients in the home, without direct supervision. This includes those with chronic disease, terminal illness who wish to die at home and complex care needs for example the physically disabled. Other settings include residential homes, nursing homes, clinic's community centres and community hospitals.

For those staff that have a dual role within practice in the ICT that extends to general practice working. Skills and competencies will be required that meet the practice nurse role.

Maintain upheld and follow the standards within the NMC code of conduct at all times ensure safe and effective delivery of patient care.

To remain up to date and within guidelines for revalidation within NMC code of conduct at all times.

Review effectiveness of nursing care provided ensuring that patient safety is maintained and care delivery meets the standards required by the Trust and Care Quality Commission.

Advise on the promotion of health and prevention of illness and provide information to individuals and groups to prevent disease, where possible. Recognise situations that may be detrimental to health for example housing, social and economic factors and refer to an appropriate agency and liaise with members of the Community Care Team.

Maintain clear and accurate records of care in line with standards required by the Trust and NMC.

The final combination of Southern Health and Solent NHS Trust services is expected in late 2024, when our new organisation, to be known as Hampshire and Isle of Wight Healthcare NHS Foundation Trust, will be formed.

Our new Trust will continue to cater to the unique needs of different communities, making healthcare across the county more accessible, as well as offering staff more opportunities for career development, training and partnership working.
