Full Job Description

To work as a member of the 24/7 Community Nursing Team - assessing, planning, implementing and evaluating care under the directionof the Team manager/ Leader. Providing nursing care to patients, of the Blaenau Gwent area, in their own homes, working collaboratively with the MDT to support a seamless high quality care provision to the patients we support in the community. Supporting the strategic direction and models of service delivery working across PCCS.Deputise for the District Nursing Team Leader/Deputy in their absenceTo act in accordance with the NMC Professional Code of Conduct and other policies

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply., To support patients clinical needs within the community a diverse clinical skill set is required. These include: diabetic management, palliative and end of life care, complex wound care, care/management of a variety of surgical drains, pressure area care/management, bowel care/management, catheter care/management, dietary care/management, health promotion, equipment provision/review, venepuncture, IV therapy, enteral feeding and gastrostomy tube care/management, tracheostomy care/management, ability to undertake verification of expected death, (or willing to work towards). Qualities required:
