Do you want to forge a better relationship with your patient and their family?Do you want to provide holistic care which is focused on your patient within their own home?Do you want to play a crucial role in an Integrated Primary care team?If the answer is yes to all of the above and you are highly motivated experienced nurse - we are looking for you to join our Salford Community District Nursing Team base at Swinton District Nurses

All staff will have a six week supernumerary induction period and support from the senior nursing team and qualified staffs that have not previously worked in the Community will have a mentor and a designated trainer to assist with the transition into the community setting.You will also benefit from personal and professional development including opportunities to undertake the Community specialist practitioner qualification, non-medical prescribing, leadership and clinical modules, and Band 5 development programme., You will work with your own caseload with full support of your Team Leader. Your caseload will be varied to your skills and training requirements. It will have a variety of patients with health and social care needs. This can vary from simple and complex patient care to looking after people at the end of their lives who wish to remain at home. You will work closely with the MDT (Social Workers, GP's and Allied Health Professionals) to ensure that patient care is appropriately coordinated.

The District Nursing Service provides care to patients and their families in their own homes and in a clinic setting.
