Full Job Description

1. To plan and manage own workload, which will be identifiedand supervised bySenior Community Nurse for Children with a Learning Children and CNCLD Team Lead 2. To share responsibility for day-to-day management of the service in the absence of the Team Leadand other team members. 3. To be a competent, autonomous community practitioner, with support from senior members of the team. 4. To work with a team of nurses and as a member of the Children and Young Peoples service to promote a seamless provision for children. 1. Toprovidecarein partnership with parents/carers/childusing interventionsappropriate tothe assessedneedas per theCNCLD Standard OperatingProcedures. 2.To evaluate the outcome of interventions using a process of regular reviewin collaboration with the child, family, CNCLDand otherprofessionals involved. 3.To advise on the promotion of health and the prevention of ill health. 4.ParticipateinSafeguardingChildrenprocesses asrequired, sharing information as necessary and completing reportswithin timescales.
