Community Nurse in the District Nurse Team

We are a looking for qualified, or soon-to-be qualified, nurse to join our Community Team based at Alfreton Primary Care Centre in North East Derbyshire. Do you want to make a difference to our patients' lives by providing evidence-based nursing care? We provide training and education for the successful candidates and are a team that likes to learn together, with regular teaching sessions and information sharing to ensure our standards of nursing are high. We can offer full-time or part-time hours to the successful candidates and are happy to be flexible to support your work life balance wherever possible. Assess, plan, implement and evaluate individualised patient care in the most efficient and clinically effective way, whilst maintaining the patient's privacy and dignity. Provide advice and support to junior staff in the delivery of high quality care and actively support the Community Nursing sister/s in the management of the health care environment. Implement / refer on to the relevant services as appropriate. Work in close collaboration with the Integrated Community Teams/GP surgeries and local acute trusts.
