Full Job Description

We are looking for a Community Learning Disability Nurse, Band 6, 37.5 hours per week, permanent contract, to work in our Community Team for People with Learning Disabilities in the Reading locality. This is an exciting opportunity to work with a wide range of people with many diverse and complex needs. This isa great opportunity to work in the community providing you with a unique opportunity to develop and learn within the learning disability service. You may already be a learning disability nurse working with people with a learning disability and are looking to take the next step into a community multi-disciplinary team, looking for a new challenge. You will be expected to do some of your visits remotely & face-to-face, working in a multi-disciplinary team approach, including working with private care providers and families. The ability to educate and influence is therefore essential. BHFT is a forward-looking Trust pursuing staff engagement.   • To manage a caseload
  • To contribute to comprehensive specialist nursing assessment
  • To promote and facilitate access to primary and secondary healthcare for individuals with a learning disability including health checks and health action plans
  • To provide specialist advice and therapeutic interventions to parents and carers of people with a learning
  • To provide and facilitate health education and promotion to include both individual and group work
