Community Nurse-Specialist Palliative Care

Meadow House Hospice is excited to announce the expansion of our palliative care service and we are looking for two dedicated Band 6 Community Nurses in Specialist Palliative Care to join our team. These are new roles for the hospice, so it offers the opportunity for staff to be at the frontline of developing and leading on the way we deliver care to our patients, their families and how we offer support to our partners in caring., Delivering an evidence-based Palliative Care Service to the adults of Ealing and Hounslow.
Working collaboratively with the Primary Care Team, acute sector, Social Services, and other agencies, to establish close working relationship that is essential in ensuring that care is coordinated and responsive to individual need.
Supporting the specialist palliative care team in managing patients and their carers ongoing needs.
Monitoring and assessing the holistic well-being of palliative care patients and their families under supervision., We promote the wellbeing and safety of all our patients and carers by acting promptly on concerns, communicating effectively and sharing information appropriately, according to the standards determined by our policy and procedures. All posts working with vulnerable groups will be subject to an enhanced criminal records check upon appointment and staff will need to give their consent for the Trust to receive regular updates on their criminal records status throughout their employment.

They must be skilled in collaborative working with a variety of healthcare professionals and will be supported by senior staff in assessing and managing a caseload of patients.
