Full Job Description

An exciting opportunity has arisen for a motivated and enthusiastic person to join our Older Adults Mental Health Community Team ., The Team provides intensive monitoring and support to older people with a variety of mental health needs.

This role is diverse and rewarding for the right individual who has a true commitment to working with older adults .

Main duties of the job

The post holder will be required to work as part of a team providing support to patients in receipt of Primary and Secondary Care Mental Health Services as defined by Mental Health Measure 2010.

Assisting individuals to achieve and maintain optimum levels of independence by providing support, choice and dignity in their daily living via assessment, implementation and evaluation of agreed plans of care.

The post holder is required to work alongside the Registered Nurse in providing and monitoring an enhanced level of support ,assessment and treatment to patients under the CMHT

Working collaboratively with the Multi-disciplinary team (MDT) to ensure the maximum benefits to patients/residents using a patient centred approach to care

Supporting the qualified staff in the observation and monitoring of medication side effects, physical observations and mental health of patients and reporting these to the MDT.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
