Full Job Description

We are looking to secure a registered Band 6 Nurse to join our established Community Nursing Team to deliver excellent health care across St Helens.,

  • To provide skilled nursing care and advice to patients in their own homes and elsewhere e.g., residential homes to undertake a comprehensive specialist assessment of patients using research to underpin their practice.

  • To demonstrate the promotion of healthy lifestyle practices to the public by the utilisation of their teaching skills.

  • Manage clinical caseloads efficiently and effectively.

  • Prioritise health problems and intervene appropriately to assist the patient in complex and urgent situations.

  • To contribute to quality improvements via clinical governance, clinical supervision, and essence of care.

  • To respect confidential information obtained during your work and refrain from discussing such information without the consent of the patient / client or a person entitled to act on their behalf, except where disclosure is required by Law or by the order of a Court or is necessary in the public interest.

  • To liaise with other disciplines and members of the Primary Health Care Team in the provision of patient care.

  • Maintain clear, concise, legible, and contemporaneous patient records always, in line with Trust Policies and Procedures and Standards for record keeping NMC.

  • Under the Data Protection Act, staff using computers will be required to ensure that security procedures are followed.

    Community experiencedadult nurse and able to demonstrate extensive knowledge, analytical and the practical skills required

  • Able to demonstrate delivery of quality care, through assessment, planning, implementation and evaluation

  • An excellent communicator / listener, be non-judgmental and empathetic with patients, carers

  • Able to work in partnership, liaising with health and social care professionals to ensure holistic patient care

  • Dependable, committed and dynamic

  • Flexible in meeting the needs of the service within the community setting

  • Knowledge in implementing change, leading service improvement

  • Committed to improve the health, wellbeing and independence of residents within the borough of St Helens


  • An Enhanced DBS with barred lists for both Adults & Children is required for this post.

Our inpatient unit at Newton Community Hospital is where patients needing acute hospital beds can continue rehabilitation, freeing up space for more unwell patients. We also provide urgent care at our Urgent Treatment Centre located in St Helens town centre.

    Specialist Regional Services

    We provide the Mersey Regional Burns & Plastic Surgery Unit at Whiston Hospital and the Spinal Injuries Unit at Southport Hospital to more than 4 million people across the whole of Merseyside, West Lancashire, Cheshire, Isle of Man and North Wales.
