This post supports the Clinical Lead and Service Manager in the management and professional leadership of all nursing team members to deliver a high-quality service in compliance with statutory responsibilities, professional guidelines and the policies and procedures of UHCW NHS Trust. This post provides day to day professional and managerial input to the community nursing team within their area of responsibility. The post holder is responsible for the delivery of the highest standards of patient centred care to patients and their relatives through the development and management of staff within their remit. The post holder will function as an independent autonomous practitioner by virtue of in-depth knowledge, expertise, proficiency, and experience and will lead and enable the team to provide a high-quality service.

  • Interviews for this post are provisionally scheduled for wc 6/01/2025, The clinical responsibilities for this role as named nurse for a Community Nursing Team will include the assessment of patient need, delivery of care and evaluation of care interventions in line with evidence-based practice for patients who require expert nursing care at home. Full UK driving licence and access to a car that can be used for business use is essential, 1. To work as an autonomous practitioner to deliver a high standard of patient care as part of the Community Nursing Team;- assessment, diagnosing, planning and evaluation of care management, using advanced clinical skills and in-depth theoretical knowledge base.
  • 2. To effectively manage a complex caseload of patients as part of a multidisciplinary team (MDT), providing specialist and evidence-based assessment and interventions to patients ensuring the physical, psychosocial, behavioural, perceptual, cognitive, sensory, and environmental needs of patients, are addressed. 3. Utilising highly specialist knowledge and clinical reasoning, to plan and provide clinical intervention in consultation with other colleagues in the multidisciplinary team and to work in a range of settings as appropriate (e.g. patients home, work environment or other community setting) 4. Responsible for day-to-day management and professional leadership of clinical staff in their identified teams 5. To participate in 7 day working rotas to ensure delivery of key services. 6. Professionally accountable for delegating tasks, mentoring, supervising and directing the work of junior registered and non-registered staff and students. 7.
