Full Job Description

Community Nursery Nurses work as members of the Healthy Child Programme (HCP) Team, across health visiting and school nursing services, in a community and public health context. The role includes:

  • developmental assessments of young children
  • family support and advice on parenting and childcare in clinic,
  • children's centres, and home settings - to individuals and groups
  • facilitating health promotion sessions in nurseries and schools
  • CNNs contribute to the public health agenda, which includes promoting school readiness, and to the integrated early help offer, in order to improve outcomes for children and families, and work closely with partner agencies in order to achieve this. CNNs undertake work delegated, and supervised by, Health Visitors and School Nurses, and may also be guided by Early Years Practice Managers. We are therefore seeking to improve the diversity of our workforce to make it truly representative of our local population., Undertake work with individual families and children in home, clinic, children's centre or school settings, as part of a plan of care delegated by the named health visitor or school nurse. Contribute to the developmental screening and monitoring of children from 0 to 5 years as part of the national Healthy Child Programme, and in line with the Greater Manchester Early Years Delivery Model. This will include use of the Ages and Stages Questionnaires and measurement of height and weight. 2 Deliver planned programmes and support groups, promoting, for example, breastfeeding, post-natal support, parenting, including parent-infant mental health, weaning, communication, and physical development. Give health promotion advice e.g. infant feeding, weaning, sleep, toilet training (including bed wetting), parenting, play and development, healthy eating, child safety, hygiene and dental health. This may be to individuals, or groups of parents and/or children in home, school, clinic or children's centre settings. Promote the free entitlement to childcare for 2, 3 and 4 year olds. Signpost parents/carers to local voluntary and statutory agencies who may be a source of extra support. Work with colleagues in the HCP team and wider agencies in order to meet the needs of children and families and Personal Care Plans (PCPs) for children with Special Educational Needs and Disabilities, and case conference reports as required. Contribute to the HCP corporate working processes including participation in allocation meetings. Be responsible for evaluating and reporting back findings and progress from work with children and families to the named health visitor or school nurse, in order to inform planning and evaluation of care and management of risk. Organise and plan own day to day work load and activities in order to meet the demands of the job role, including arranging home visits and ensuring that planned clinic/children's centres and school sessions are covered. Create displays and information resources on health promotion topics Participates in training and development activities that occur within the HCP and Early Years environment and are relevant to the role. Ensure safe and efficient use of equipment, resources and consumables at all times. Ensure stock levels are maintained e.g. of ASQ resources. 3 Responsible for the requisition, maintenance, cleanliness and safety of toys and equipment, as needed by the team. Engage in service development as part of the HCP team; contribute to the development of standard procedures and local policies. Engage in all relevant recording and evaluation processes for interventions, programmes and support groups. Contribute to audits, data collection and research projects as required. Demonstrate own role and support the learning of new staff and students. Responsible for delegation of tasks to generic support workers/ health care assistants., Be responsible at all times for own actions, and be accountable for actions to named health visitors and school nurses Plan, organise and deliver open baby clinics and developmental screening sessions, including managing waiting lists, arranging appointments and follow up visits, and onward referrals. Work with colleagues in local authority Early Years services in delivery of interventions as part of the Early Years Delivery Model (Grow). Able to identify additional needs e.g. for specialist services and refer on e.g. to Communication or Move and Play groups, speech and language therapy, audiology or gait clinic. Be aware of issues of equality, diversity and culture, and potential barriers to inclusion and communication and adapt care effectively. Communicate effectively through a range of media, adapting communication to meet the needs of the listener/reader. Maintain patient records to the required standards using both paper-based and electronic systems and comply with Trust requirements for data collection. Able to record and plot growth measurements on centile chart and recognise when measurements that deviate from normal require a referral to Health Visitor or School Nurse. Demonstrate skill in measuring and weighing neonates with great accuracy because of potential to be required for diagnostic and evidence purposes. Observe confidentiality and information governance requirements at all times. Work is mainly clinic based, but also entails travelling for home visits and work in community settings e.g. children's centres. Setting up clinics and preparation of rooms for sessions may entail moving furniture, transporting equipment e.g. scales, resources, records. Working with young children may entail bending and stretching, kneeling, moving and lifting e.g. getting in and out of car, carrying equipment, putting out of mats, cushions, putting up displays. 5 Home visiting and work in the community may entail exposure to inclement weather, poor standards of hygiene, pets and other hazards. Working with babies and young children may entail exposure to urine, faeces, vomit and head lice. Concentration required, sometimes in noisy or distracting environments, e.g. when undertaking assessments, recording results, report writing Accuracy is essential when conducting and recording measurement of height and weight There will be some exposure - indirect and occasionally direct, with distressing and sensitive situations/ information such as child protection, disability, terminal illness, sudden unexpected infant death, still birth, premature birth, domestic violence, drug and alcohol misuse, and mental health problems. Such issues have an impact on behaviours of families, for example depression, aggression, and unwillingness, lack of motivation or reduced capacity to engage with services. CNNs may be in situations where they are the first to identify a specific need e.g. child protection and are expected to respond to the immediate situation and refer back to the named health visitor or school nurse. 1. To provide cover for colleagues as directed by your manager. 2. Risk Management (Health & Safety) a) You will follow risk management procedures at all times. The Risk Management procedures for the Trust and the department are kept by the departmental manager. b) You are personally responsible for Risk Management issues in respect of yourself and your colleagues. c) If you identify a potential hazard you should report it to your manager / supervisor at once. If in doubt you should speak to your manager for guidance. d) You must always use safety equipment provided, and report any defects to your manager. You must attend Risk Management Training as directed by your manager. 6 e) If you are a manager or have line management responsibilities for staff or a department or area of work, you are responsible for the Risk Management issues in that area. You will ensure that there is an annual Risk Management audit in your area and make sure that where necessary, an action plan eradicating risks is drawn up and implemented. f) Should you need help in resolving Risk Management matters, you must seek assistance from your manager. 3. Infection Prevention and Control a) You are personally responsible for ensuring that you protect yourself, patients, visitors and colleagues from the risks of infection associated with health care activities and the care environment. b) You must adhere to infection prevention and control policies at all times liaising with the infection control team and acting on any instructions given. c) You must attend regular infection prevention and control update training. d) You should at all times promote and demonstrate good practice for the prevention and control of infection. 4. To avoid any behaviour which discriminates against your fellow employees, or potential employees on the grounds of their sex, sexual orientation, marital status, race, religion, creed, colour, nationality, ethnic origin or disability. 5. To safeguard at all times confidentiality of information relating to patients and staff. 6. No Smoking Policy The Trust operates a No Smoking Policy which states that smoking is prohibited within all Trust premises and on the site. This includes entrances, exits, cars, lease cars, car parks, pavements and walkways, areas hidden from general view and residences. As an employee of the Trust you are expected to comply with this policy, failure to do so may result in disciplinary action being taken under the Trust's Conduct and Disciplinary Policy. 7. To behave in a manner which ensures the security of NHS property and resources. 8. To abide by all relevant Trust Policies and Procedures.     Demonstrate in-depth knowledge of child health and development, including the development of healthy parent-infant / child relationships and parental well-being, parenting styles, child development and child care. Demonstrate knowledge of relevant public health priorities, including school readiness, and the contribution that health and early years services can make to improving outcomes for children and families. Demonstrate knowledge of local services, health and other agencies, early years provision and current guidance from best practice and legislation. Demonstrate sound knowledge of safeguarding and child protection issues. Follow the Trust child protection procedures and policies and the Local Safeguarding Children Board guidelines. Demonstrate understanding of Special Educational Needs and Disability (SEND) and the importance of early identification and access to services, including close liaison with children's centres, pre-school settings, childminders and schools. Be able to work independently, carrying out delegated duties, under the supervision of a qualified health professional
