Full Job Description

You will be part of the Ambulatory Clinic Team that covers the South East Alliance which includes Burntwood, Lichfield and Tamworth. The service provides treatments and assessments to non-housebound patients living within our community. As a Band 6 within the team, you will be responsible for allocating planned appointments, triage new referrals received from our Local Access Point and prioritising those who need appointments sooner than others and be a leader within the team in order to support our Band 5's, 4's, 3's and 2's. We are a proactive team, encouraging self care to those who visit our clinics and you will also manage a caseload ensuring that holistic assessments are robust, and that patients are managed appropriately.

A vacancy has arisen with the South East Ambulatory Clinics for an enthusiastic individual to join our Clinic Team providing care to clients in various clinic settings across Burntwood, Lichfield and Tamworth. You should have excellent communication and interpersonal skills and the ability to organise and prioritise your workload without direct supervision. Along with other Band 6 colleagues, you will support the Operational Manager in managing and coordinating aspects of care and service provision including case management within the team. Past experience within a community setting is essential. You must have excellent leadership skills and possess the ability to lead a team. You will be supported by the Lead Specialist Practitioner and Operational Manager and through peer support from other Band 6 District Nurses within the team. There will opportunities for personal development through informal and formal learning, including the opportunity to complete V300 Prescribing Course if not already achieved.
