Full Job Description

- Can you lead a team of nursing staff compassionately and inclusively? We are seeking people who are adaptable and collaborative, with the potential to grow, and the ability to plan, prioritise and organise their work, to join our neighbourhood nursing service. The neighbourhood nursing teams provide nursing care for patients, aged 18 and over who are housebound within the Medway area, delivering a range of personalised care and treatment to those with complex and long-term conditions, supporting people to stay safe and well at home. You will be a registered nurse or paramedic responsible for patients allocated to them and will assess, manage, plan, and deliver care, including being professionally and legally accountable. You will lead and manage a Neighbourhood Nursing Team, geographically aligned to a locality and be responsible for the management of a caseload ensuring the provision of a high standard of nursing care to patients and their families in the community. You will work as part of the integrated health care team, working within and across professional and organisational boundaries. liaising with GPs, other professionals and statutory and voluntary agencies, they will deliver individualised care to patients. The service operates a 7am to 7pm service, 365 days in the year. You will need to have a District Nursing qualification / community practitioner pathway or be willing to commence this training within 1 year of starting. You will also need to have access to a vehicle for work purposes.

· Recognised degree in adult nursing · Registered Nurse on Part 1 of the Nursing Midwifery Council register · Evidence of further study · Mentorship or training qualification · District Nursing qualification / community practitioner pathway or willingness to commence within 1 year of starting · Nurse prescriber or willingness to undertake training · Relevant post-graduate qualification, e.g. in elderly care, chronic disease management, long term conditions, rehabilitation Experience · Post graduate accumulative experience in the fields of elderly/ rehabilitation/ community/ intermediate care/ long term conditions/acute illness. · Experience of delivering education and teaching. · Proven and evidence of experience in an autonomous practitioner role · Experience of working in the community in healthcare. · Experience and competence in high-level clinical assessment of complex patients · Evidence of specialist knowledge in minor illness management, community nursing and advanced practitioner · Experience of using technology to facilitate provision of patient care · Experience of motivational interviewing · Change management experience Special knowledge / expertise · Knowledge of current issues and trends within community healthcare, urgent Care, long term conditions, intermediate care and the NHS in general · A working knowledge of the admission avoidance ethos and facilitation of timely discharge · A sound knowledge of clinical governance and risk management · Ability to practice at an advanced level · Experience of lone working and decision making · Experience of mentoring students and other healthcare professionals · Proven ability to manage a caseload · Intermediate IT skills (Windows, Internet, Outlook, Excel, EPEX) · Presentation skills and teaching qualification or experience · Research awareness and audit skills · Experience of using research and other evidence to inform practice Disposition, adjustment, attitude and commitment · Self-motivated · Able to work and make decisions autonomously and take the initiative · Assertive but diplomatic towards colleagues, patients, carers and relatives with compassionate and inclusive interpersonal skills which will encourage self-empowerment · Flexible and responsive to the needs of the service · Ability to recognise own limitations and to refer appropriately when necessary in accordance with professional code of conduct Practical / intellectual skills · Excellent verbal and written communication skills · Effective time management, organizational skills and ability to meet deadlines · Able to travel to dispersed sites in a timely manner MCH values Being caring and compassionate, Disability Confident About Disability Confident A Disability Confident employer will generally offer an interview to any applicant that declares they have a disability and meets the minimum criteria for the job as defined by the employer.
