Full Job Description

The Salford Adult Social Care (ASC) Therapy team provide the people of Salford (including children) with assessments to support them to be as independent as possible and safe at home for as long as possible. Recommendations following assessment are often funded by the Disabled Facilities Grants (DFG) and therapists have access to the latest in equipment to support assessment outcomes.

Our team has an excellent reputation for innovative working and the Therapists are specialists when it comes to moving and handling too.

You role will involve screening new referrals as they come into the service, carrying out functional assessments within a person's home environment, to identify risks to people within their own home or at times in a hospital setting to aid discharge.

You will use your skills and knowledge around specialist equipment, moving and handling, complex housing adaptations and the use of the DFG. The Community Therapy Service offers a comprehensive induction, regular supervision, and CPD / learning opportunities to support your role and development. Even if you have limited experience or understanding of the role of OT in Adult Social Care, but you are interested to learn, we would really love to hear from you as we can support your development.,

  • To be responsible for the screening, prioritising and allocation of referrals into the service.

  • To contribute to the overall development of the service

  • To be responsible for carrying out Occupational Therapy duties in relation to complex cases requiring a high level of skill, knowledge, and professional expertise

  • To work as an autonomous practitioner in the community, being professionally and legally accountable for all aspects of your work, managing your own caseload and maintaining patient and own records.

  • To assist the Operational Lead, Team Manager and B7s, in ensuring the delivery of effective services through the promotion of high standards of practice in assessment and care planning.

  • To work collaboratively with other disciplines, providing positive Occupational Therapy support and advice to enable independence and quality of life.

  • To undertake responsibility for managing specific aspects of practice at the direction of the B7s and Team Manager.

  • To provide clinical supervision, as per the team structure for other members of the team. Also, to supervise individual students and undertake the practice educator role and to provide assessment and support to other staff taking students on placement with the service.

  • A degree or diploma in Occupational therapy is essential along with state registration with the Health and Care Professionals Council (HCPC)
