Full Job Description

A Vacancy at University Hospitals Coventry and Warwickshire NHS Trust. An exciting opportunity to join the Night Rapid Response Palliative Care Team. Coventry Palliative Care Team supports patients, families and carers with specialist palliative care needs, palliative support, and end of life care within the community setting. The post holder will be required to work as part of the integrated palliative care multidisciplinary team and will work with other health care professionals to provide specialist advice, support, and high-quality patient centred care to those affected by malignant and non-malignant disease at the end of life and generalised unplanned nursing care within a 2-hour rapid response. This will be provided 7 days a week 8pm-8am on a shift rota basis. Please note that a Full valid UK driving license and access to a vehicle that can be used for work purposes is essential for this vacancy.

  • To provide direct assessment and advice for patients with cancer or an advanced terminal illness and their families, who require specialist palliative care.
  • Triage and accept referrals direct from other disciplines, patients and carers and refer to other agencies where appropriate.
  • Responsible for the coordination and provision of unplanned care to patients and their careers as part of a 24-hour nursing service.
  • To act as a specialist resource for community and primary care colleagues in the management of patients with specialist palliative care needs and urgent generalised nursing needs
  • Contribute to the development and review of policies, procedures and protocols within the Trust related to palliative and generalised care.
  • Actively manage unscheduled visits to patients in a 2-hour rapid response timely manner.
  • Undertake comprehensive risk assessments associated with the care of patients to ensure nurses' and carers' safety
  • To safely refer patients on to the relevant health care provider as and when appropriate
