So if you want be a part of this, we would love to hear from you., Under the direction and supervision of the Therapy Clinical lead for Community Rehab the post holder will undertake a team lead role contributing to the planning, coordination, delivery and evaluation of the Community Physiotherapy and Community Rehab team as a whole
The primary purpose of this role is to assess, treat, and support patients with complex physical conditions, helping them to achieve optimal functional independence and quality of life. This position requires a high level of clinical expertise and the ability to work autonomously within the community, managing a diverse caseload of patients across various age groups and with a range of conditions (e.g., neurological, musculoskeletal, respiratory, and age-related issues).
Through the application of advanced clinical reasoning and differential diagnosis skills responsible for the formulation of accurate clinical diagnoses and treatment indicated advising the patient, carers/ relatives and other AHPs appropriately.
Responsible for prioritisation/ triage of patient referrals, identifying any serious pathology and managing appropriately., Clinical Responsibilities:
Conduct thorough assessments of patients in their home or community settings, including those with complex and long-term conditions.
Develop and implement individualized treatment plans to address physical impairments, functional limitations, and rehabilitation goals.
Provide physiotherapy interventions to promote mobility, strength, and overall physical health.
Offer advice and education to patients and caregivers to manage conditions, prevent injury, and maintain independence.
Use evidence-based clinical reasoning to monitor progress and adjust treatment plans as needed.
Work closely with other healthcare professionals (GPs, occupational therapists, social workers, etc.) to provide holistic patient care.
Provide rehabilitation support for patients recovering from surgery, injury, or illness.
Manage a caseload of patients with diverse needs, including older adults, those with non-complex neurological conditions, musculoskeletal disorders, and orthopedic issues.
Leadership and Supervision:
Provide clinical supervision, mentorship, and support to junior physiotherapists, students, and support staff.
Lead and manage case discussions, ensuring a collaborative approach to care.
Act as a role model for best clinical practice and professional development within the team.
Support staff with complex case management and decision-making, such as safeguarding concerns.
Contribute to the development of clinical protocols and policies within the service.
Service Development:
Identify opportunities for service improvements and participate in the development of community physiotherapy services.
Implement and evaluate new clinical pathways and evidence-based practices.
Participate in audit, research, and quality improvement initiatives to enhance service delivery.
Assist in training and development programs for staff to ensure high standards of patient care.
Administrative Responsibilities:
Maintain accurate and up-to-date patient records, adhering to confidentiality and data protection policies.
Participate in team meetings, case reviews, and planning sessions to ensure continuity of care.
Contribute to the production of reports and documentation for clinical governance and service delivery evaluations.
Support budget management by ensuring efficient use of resources and equipment.
Degree or equivalent in Physiotherapy.
Registration with the Health and Care Professions Council (HCPC).
Evidence of continuous professional development (CPD), including relevant post-graduate training and courses.
Significant experience working as a physiotherapist, with proven expertise in community settings.
Experience in managing complex cases and working autonomously in the community.
Excellent communication and interpersonal skills to liaise with patients, caregivers, and multidisciplinary teams.
Ability to supervise and support junior staff and students.
Desirable Qualifications:
Post-graduate qualifications in relevant areas
Experience with service development, audits, or research projects.
Knowledge of community health services and related pathways.

As the role is community based having access to a car everyday for work is essential to this role.
As the band 7 community physiotherapist, you will play a key role in providing high-quality physiotherapy services to patients within Medway. Working as part of a multidisciplinary team, you will assess, diagnose, and treat individuals with a wide range of conditions, promoting rehabilitation and improving physical function. You will take on leadership and supervisory responsibilities, supporting other members of the team and students.
The community physiotherapy team in Medway is part of the wider community rehabilitation team which encompasses the falls team, walter brice and neuro outpatient physiotherapy team.
