Full Job Description
Great Western Hospital Community Podiatry service prides itself on an integrated approach to support our patients and colleagues. Working within the integrated service you will have the opportunity to work closely with our community nursing teams. You will be supporting patients with complex needs within a clinic setting or their own home and there will be a requirement to complete a competency framework .
This service will be transitioning to HCRG Care Group from the 1st April 2025. The successful candidate may transfer over (under Transfer of Undertakings (Protection of Employment) Regulations 2006) on the terms and conditions they are employed on. Great Western Hospitals NHS Foundation Trust is committed to ensuring a smooth transition for everyone involved.
Main duties of the job
To work as part of the integrated podiatry team, providing care in accordance with service criteria. This will include working independently in clinics, hospitals, GP practices and domiciliary settings, providing high quality clinical care in accordance with NICE Guidance.
To assess, diagnose, develop and implement individual treatment plans for patients with a wide variety of clinical needs. Most commonly, care will be provided for patients at risk, who suffer from metabolic, vascular or neurological disorders e.g. diabetes compromising the integrity of the lower limb.
