Full Job Description

The Community post covid service has expanded its service offer to Enfield residents and is now looking to recruit a Rehab assistant to join the team. This is an exciting opportunity to support the development of virtual programs delivered across the three boroughs for residents in Islington, Haringey and Enfield. The Community Post Covid Service is a multidisciplinary service that supports people living with Long Covid. We use a personalised care approach to help people to learn more about their symptoms and develop skills and strategies to support their recovery and reduce the impact of living with ongoing symptoms that affect day to day function. The role of the Rehabilitation Assistant will be to support the CPCS team to deliver services to people living with Long Covid with key responsibility to assist with the virtual group programme including monitoring wait list, group set up, assisting clients with MS Teams and supporting the clinical team to deliver therapeutic group content. The successful candidate will have excellent organisational, communication and time management skills with are vital to the role. The candidate will work within a department that is committed to evidence-based practice and service development and improvement.,

  • To work within a multi-professional team and provide support to a team of health care professionals.
  • To report any changes or concerns in a patient's condition to a member of staff. To support specialist nurses in clinic settings and on home support/case management which may involve domiciliary visits to patient's homes. To implement treatment and care to patients under the guidance and supervision of a registered professional. To communicate accurately and effectively with GPs and referrers regarding patient referrals following clinic appointments. To record activity on a clinical database as required and maintain accurate and contemporaneous clinical records of given care. To be responsible for administrative support within the team as required. Communicate effectively with a range of people, either in person or on the telephone, promptly and in a professional and courteous manner whilst recognising barriers to communication. Respond to enquiries, and take appropriate action on behalf of the service, either by telephone, personal contact, or correspondence, within agreed time limits.
