Community Practitioner

The post holder will be a Registered Adult Nurse or HCPC Paramedic who will have significant intermediate care experience at Band 5 or 6. Must possess Clinical Assessment Skills and either Non-Medical Prescriber or working towards qualifications. The post holder will demonstrate safe clinical decision making and expert care for the patients registered within the service. The post holder will be professionally and legally accountable for all aspects of care from initial history taking through to treatment. They will be responsible for patients allocated to them and will assess, manage and deliver care to meet the complex needs for those patients requiring intermediate care and primary care services.
