Community Psychiatric Nurse

The Community Psychiatric Nurse will be a member of the Older Adult Community Mental health team, working closely with the multi- disciplinary team. Responsibilities include the maintaining of clinical caseloads and conducting assessments of highly complex problems which require analysis, interpretation and comparison of a range of solutions for service users who are experiencing severe and complex mental health difficulties, both organic and functional., The Community Psychiatric Nurse will work within their code of practice to take responsibility for their allocated caseload to ensure assessments of health and social needs. This will include formulation, planning and evaluation of care which include regular reviews and update of risk assessments documentation and care planning. They will be a care co-coordinator and advocate for the service user at times and promote independence through the recovery process.

The ability to speak Welsh is desirable for this role; Welsh and/or English speakers are equally welcome to apply.
