Full Job Description

What youll be responsible for : Providing robust assessment of patients (by telephone or face-to-face) who have beenreferred to the CMHT due to presenting with mental health difficulties. Identifying appropriate care pathways and ensuring the least restrictive option to meet theneeds of the patient and ensuring their wellbeing. Signposting to appropriate care providers (including primary and tertiary care). Attending dailyRed Board meetings. Raising concerns to senior staff for support and management. CompletingRiodocumentation and planning care for patients. Presenting cases to Multi-Disciplinary Team for discussion around care pathway. Liaison with colleagues nursing, occupational therapy, medics, psychology and admin, This flexible role as a Community Nurse for NHS Professionals offers a unique blend of professional fulfilment and adaptable scheduling.   • Identifying appropriate care pathways and ensuring the least restrictive option to meet the needs of the patient and ensuring their wellbeing.

  • Completing documentation and planning care for patients.
  • Presenting cases to Multi-Disciplinary Team for discussion around care pathway.

  • The role will involve, but not be limited to you performing the above.

  • A minimum of 6 months paid experience as a Community Psychiatric Nurse in an UK hospital environment within the last 3years
  • Comply with NMC Professional Registration and keep up to date with current nursing practices
  • Full UK driving licence and own vehicle.
  • Depending on the Trust, you may require variable additional certification and skills., A minimum of 6 months paid experienceas a Community Psychiatric Nursein anUKhospital environment within the last 3years., Essential

  • Degree / Diploma in Nursing
