Full Job Description

Slough OPMH has an exciting opportunity for a Community Psychiatric Nurse who wishes to join our dynamic Home Treatment team. You will work alongside a very experienced clinical team and take responsibility for the assessment and implementation of intensive care and treatment in the home of people with mental health needs, as well as people with dementia. You must have specialist knowledge of the care of older people with mental health problems and mental health crises. You will be required to travel within the community for the performance of your duties. The service operates 7 days per week, so flexibility with regard to unsocial hours/ shift patterns will be required. We are a friendly, dynamic and supportive team. You will be actively encouraged to develop professionally in your role within the team, and have regular access to supervision and peer support in your role.,

  • To ensure the correct application of Mental Health Clinical Risk policies and procedures.
  • To demonstrate a good level of knowledge in the assessment of the cognitive, psychological, physical and social functioning
  • To develop and implementappropriate individual plans of care and regularly evaluate these with the Home Treatment Team,patient and carer where appropriate.
  • To provide a fast and flexible response to care needs, responding to referralsin a timely manner and providing intensive treatment (up to three times daily).
  • Toparticipate in individual and/or group sessional work designed to meet the emotional and psychological needs of patients.
  • To be knowledgeable of all necessary resources within the community toassist in the implementation of care plans.
  • To take responsibility for being an active team member byparticipating in regular staff support meetings, review meetings and team building exercises.
  • To communicate formally and informally with team members, other colleagues from the multidisciplinary team, social services, primary care team, voluntary sector and carers.
