Full Job Description
The role of Community Nurse is to work in partnership with service users in the designatedcare group with complex health presentations, developing their care plans and recovery plansto enable them to lead fulfilling lives. They will demonstrate an understanding of clinicalinterventions relevant the care group to include education, care plans and discharge planning,working closely with carers, families and partner agencies to facilitate this. The post holder willwork at all times to promote the safety and the well being of service users and theirfamilies/carers. The post holder will assess, plan and implement care, and provide specialistnursing advice and carry out specialist nursing procedures. They will provide clinicalsupervision to staff and students.
We provide mental health and learning disability care for children, young people and adults across Sussex and for children and young people in Hampshire. We have an exciting opportunity to offer the right person to be a part of our committed and enthusiastic team working with people who have complex needs relating to their diagnosis of dementia and mental health illness. Our service is in the process of moving away from a more traditional model to adopt the follow me care principles advocated in NHS England's new Community Mental Healthcare Strategy. We are confident that this approach will help us to meet the growing demands for services whilst continuing to provide good outcomes for people. This post will sit within our multidisciplinary community dementia team and will work as a lead practitioner to deliver interventions to those people who have more complex or intensive needs. We aim to help people to remain safe and well at home and able to maintain the relationships that are important to them. The service has an intensive support element within it that offers extended hours and weekend interventions to people who are seen by the team. The service operates 0900 - 1700 hours Monday to Friday Applicant must be able to drive and have access to a vehicle for work.,
