Full Job Description

Within the caseload, and as delegated by the Specialist Community Public Health Nurse (SCPHN), the post holder will contribute to the safe and effective delivery of the 0-19 service. Working alongside the wider team you will promote children's physical and emotional health and wellbeing, identify health, social or developmental concerns at the earliest opportunity and contribute to the aim for all children to have the best possible start in life. You will be based within the locality (neighbourhood) and will work with the 0-19 teams in that locality and supporting county wide work where needed. Under the direction of the SCPHN (HV/SHN) the role involves working in partnership with children, young people, families, primary care, schools, statutory and voluntary services to facilitate health-enhancing activities and improve health outcomes. You will deliver services to children and families within homes, and community settings. Under the delegation of a SCPHN you will work with children 0-8 years and implement and evaluate episodes/packages of care according to the individual needs of the child/family. You will also run parenting groups such as "Early Days," and "Marvellous Me" and will deliver individual and group developmental reviews, and be involved, under supervision of the named SCPHN band 6, with complex situations (see detailed job description). The role will require the post holder to adhere to Safeguarding policies.
