Community Public Health Nurse

Within the caseload, and as delegated by the Specialist Community Public Health Nurse the post holder will contribute to the safe and effective delivery of the 0-19 service.
You will be based within the locality (neighbourhood) and will work with the 0-19 teams in that locality and supporting county wide work where needed for children aged 0-8 years. The role involves working in partnership with children, young people, families, primary care, schools and statutory and voluntary services and other partner agencies to facilitate health-enhancing activities and improve health outcomes.
You will deliver services to children and families within homes, and community settings.
Clinical activities will include assessing, planning, implementing and evaluating family focused care under the delegation of a SCPHN Health Visitor/School Health Nurse and working in partnership with other agencies.
The role includes undertaking Healthy Child Programme developmental and transitional assessments and planning, delivering and evaluating public health initiatives through community-based health promotion activities.
The role will require the post holder to adhere to Safeguarding policies and actively engage under the supervision from a SCPHN in safeguarding processes, including Early help assessments and Team around the Family processes.
You will work with partnership with other agencies and develop, participate and maintain interagency networks., + All new starters have a thorough induction process, both local and Trust wide, which aims to create a positive supportive working environment allowing new employees to learn key elements of their role over a reasonable timescale.
