Then why not apply for thisBand 5 Community Public Health Nurse (CPHN) post, working with children 0-5 years, with Kent Community Health NHS FoundationTrust, and come and join our vibrant and forward thinking team.We have full and part time opportunities ranging from a minimum of 22.5 hours to a maximum of 37.5 hours per week between 9amto 5pm. This role is a community-based role so the majority of your duties will be carried out between your base and communitysettings or client homes., Working as part of the Health Visiting Service you will support the delivery of the Healthy Child Programme in accordance withnational and local strategy/guidance, a key part of your role will be to undertake universal and targeted assessments, mandated well-being reviews and packages of care in line with evidence- based practice, recognising when to escalate concerns to the appropriatequalified Health Visitor or District Manager. This role requires you to undertake regulated activities to both Children and Adults.You will play a vital role in promoting and protecting the public's health and this might be achieved by promoting healthy lifestylesand reducing the likelihood of ill-health, or preventing illness through immunisations or screening interventions or identifying needsrequiring more specialist intervention.
