Full Job Description
As a Community Reablement Worker in the Learning Difficulties & Autism Team, you will deliver a community-based enablement and inclusion service, which supports people to learn or relearn skills, reduces social isolation, and encourages young people to become part of their community and as independent as possible. You will work with individuals as they transition to adult services and the key people in their lives to identify specific goals and outcomes which you will then translate into a progression plan, setting out how these will be achieved and including strategies that can be used to manage any identified risks. As part of your role, you will support individuals to explore a range of community options such as access to employment, voluntary opportunities, recreational/social pursuits, and community, cultural or spiritual activities. Where necessary, you will provide information to commissioning workers about an individual's eligible needs as well as identifying and reporting potential safeguarding, mental capacity, and other high-risk situations where a person's safety or liberty may be at risk. This is a part time, 18.5 hours per week post. A DBS enhanced check for a regulated activity is required for this post., We are keen to support employees to balance their working life with other commitments. - Hold a relevant health/social care qualification to NVQ Level 2 or equivalent or have equivalent relevant experience.
- Have relevant experience of working with adults with a Learning Disability and/or Autism, individuals who have mental health conditions and physical and a social care setting and of working with other agencies.
- Have an understanding of recovery, social inclusion, self-directed support, and person-centred planning as well as knowledge of a variety of conditions and how these can impact people's lives.
- Have a good understanding of the services that are available from external parties such as the NHS and community agencies and an awareness of how social care organisations fit within the wider community/voluntary sector.
- Have knowledge of risk assessment and be able to work with individuals to develop strategies to manage any identified risks.
- Have good communication, prioritisation, time management, and IT skills.
- Be able to travel to various locations throughout the county in a cost-effective manner.
