An exciting opportunity has arisen for a Community Recovery Worker to join the new and developing Home Engagement and Recovery Team (HEART).
HEART aims to provide bespoke, intensive, and individualised interventions and care to service users living with complex, enduring mental health problems to enable them to live fulfilling lives in the community.
The post holder will promote the independence and well-being of service users; assist social inclusion and enable the widest possible expression of each individual's chosen lifestyle., As a recovery worker you will be working intensively with service users, enabling them to live their lives and achieve their own goals - with support from a multi-disciplinary team of nurses, occupational therapists, social workers, psychologists, and medical staff.
The post holder will support individuals in their recovery through using a variety of interventions to promote independence, functioning and social inclusion.
Interventions may involve activities of daily living, vocational and educational aspirations, development of family and social relationships, attending appointments, accessing meaningful activities, housing related support as examples.
The post holder will support services users in a variety of settings including their own homes, supported accommodation providers and especially in the community.
The post holder will need to provide flexible working pattern that meets the needs of the service, which operates 8-8 7 days per week
