Full Job Description
They step in to support people who are either discharged from acute hospitals or experience an acute change in their function in their own home, and need short term/interim enablement support at their homes. The teams are committed to supporting people at home and work over a seven day rota.
What are the hours?
The shift pattern is Early-0800-1400 and Late 1600-2200 over a seven day rota and we have full time hours available (36 hours) along with part time options minimum of 24 hours per week., The successful candidate is expected to work and document to a high standard and work in a way that reflects the DCHS values.
To maintain standards, ensuring quality of patient care through the development of a range of knowledge and competencies to support clinicians.
To assist all clinicians in delivering agreed programmes of treatment and care where appropriate and work as an effective member of the wider multi-disciplinary team.
To carry out individual support without direct supervision in the community.
To provide support to patients to optimise their health and independence.

Working in this team means you will need be able to meet the travel requirements of going to people's homes, so you must have access to a vehicle which can be used for work purposes and have a current driving licence valid for the UK. DCHS has a mileage policy and car lease scheme available to support our staff.
