Full Job Description

Location of post : Neath Civic Centre but also work under the Local Authorities Hybrid Working Policy. The Complex Disability Team are looking a Community Social Worker, to join an established team that has recently expanded to meet the demand of our service, striving for the work force to have manageable caseloads, that ultimately goes towards making a difference to the individuals we support. We support individuals to meet their personal outcomes whilst taking into consideration of their complex health and social care needs. We are a pan disability team that typically support individuals who have learning and physical disabilities, acquired brain injuries, and neurodiversity. In addition to this, the team are involved in a critical time for young people transitioning from Children to Adult services when they turn eighteen. What we can offer you; You will receive a big welcome from a team that has adapted an ethos of supporting one another in what is arguably a challenging profession. This includes regular supervision, as well as a case discussion forum that includes colleagues from the Commissioning Team and Housing Officer. We also hold regular practice development sessions as well as case mapping exercises which are facilitated by the Consultant Social Workers. This approach to learning has proven to be a popular forum, to build on our knowledge that ultimately benefits the individuals we support. This will enable you to have an informal discussion and to answer questions they may have, before making the decision to apply. NPT will invest in their staff with opportunities to enhance their learning by providing a comprehensive training programme. For more experienced social workers there will be opportunities to train as an Approved Mental Health Practitioner, Practice Educator, as well management and leadership training courses.

  • Ideally you will have;
  • Applicants will need to be social work qualified and professionally registered as a social worker with Social Care Wales. Experienced or newly qualified practitioners are welcome to apply. You will need a knowledge and working practice of the Social Services and Wellbeing (Wales) Act 2014, to include outcome focused assessment and safeguarding. Also a working knowledge of the Mental Health Measures (2010), Continuing Health Care, the Mental Capacity Act 2005 including Deprivation of Liberty Safeguards.
