Full Job Description

To provide the specialist nurse function in the delivery of care for patients who have been referred to the Community Bladder & Bowel Service. To provide high quality patient care and assessments, working with the wider multi-disciplinary team.

To deliver training both formally and informally within the specialist field of paediatric bladder & bowel conditions.

To act as role model ensuring practice is up to date and delivered in a professional manner.

To deputise as required in the absence of the clinical Team Lead to ensure efficient and effective service delivery.

Main duties of the job

Work autonomously to manage own caseload of patients whilst working as part of the multi disciplinary team.

Undertake full patient holistic assessment, examination and clinical history.

Plan specialised programmes of care and provide specialist advice to the multidisciplinary team, patients, relatives and carers.

Ensure patients are placed in the correct treatment pathway and ensure necessary investigations and treatments are organised.

Enable patient choice and involvement, co ordinate patient pathways and liaise across organisational boundaries.

Provide clinical advice in accordance with current evidence based practice, national and local protocols and pathways.

Participate in Nurse led clinics, consultant led clinics (as appropriate) and multidisciplinary team meetings.

Participate in case conferences & meetings in relation to child safeguarding plans, formulating up to date reports to support implementation of plans of care for these children and their carers/families

Ensure patients, families and carers are given appropriate oral and written information regarding planned care
