We have an exciting opportunity for Band 5 enthusiastic, motivated, and compassionate nurse to join our established multidisciplinary Care Community Team to support and deliver a range of quality nursing care to our community patients.

Full time and part time positions available, Our care community teams is based in Princeway Health Centre covering Frodsham and surrounding geographic area . It's important that you can drive and have access to your own vehicle for work. But of course, mileage expenses are reimbursed in addition to your hourly rate.

As a Community Staff Nurse the successful candidate will undertake duties involving the delivery of high-quality nursing services and the holistic assessments of patients ensuring a person-centred approach.

You will work alongside various professionals to provide quality and seamless care to patients predominantly in their own home. You will be proactive in coordinating their care needs as appropriate, avoiding unnecessary admission to hospital or long-term care settings.

CWP provides health and care services for local people, including mental health, learning disability, community physical health and all-age disability care - including the provision of three GP surgeries.
