The Integrated Care Team are looking for Community Staff Nurses to join our Teams in Wakefield and our surrounding areas. The team covers 24/7-365 days per year. Working as part of our team, you will have the opportunities to: develop or extend your clinical knowledge and experience by working across a range of clinical settings; develop extensive clinical assessment and case management skills in areas, such as hospital avoidance, managing long term conditions, wound management and end of life care; and to plan and deliver complex care plans. We will support you to develop through our comprehensive preceptorship and mentorship programs. We have excellent electronic facilities to support your learning and development and have recently developed an online portfolio to support your revalidation., The post holder will be responsible for the planning and delivery of patient care within Adult Community Nursing Teams. The post holder is responsible for maintaining and delivering high standards of care. These services will be provided in accordance with the Trusts declared values and principles ensuring the delivery of quality, patient centred care.
- To supervise and lead staff within the District Nursing Team in the absence of the District Nurse/Caseload holder promoting positive staff behaviour and facilitating the delivery of treatment and rehabilitation to individuals, groups and communities.
- To assist in supporting the coordination of community nursing services across Wakefield District, to ensure uninterrupted, seamless patient care, to support nursing colleagues in the provision of a high quality, 24 hour, 7 day per week nursing service.
- Demonstrate the appropriate supervision skills with mentorship, achievement of learning competencies and ongoing training and development. Take responsibility for the delegated Appraisals and achievement of learning competencies for a designated number of support workers.
